Highest level of education attained

 

Check the box next to the highest grade level completed. Example if the client is currently in 7th grade, the highest grade level completed would be 6th grade, so you would check the box that corresponds to 6th grade.

 

Is the client required to attend school?  A person under the age of 18 is required to attend school unless the minor has been emancipated. 

 

 

For the following list, indicate how many weeks the client was enrolled in these educational settings during the past 12 months (enter number of weeks and average number of hours per week:

 

Not in school of any kind

High School/GED Preparation/Adult Education: Includes Continuation High School.

Technical/Vocational School

Community College/4 year college

Graduate School

Alternative Educational Setting: Home Schooling, Distance Learning (e.g., Internet)

Other

 

-Total number of weeks need to equal or greater than 52 weeks.  If a client was concurrently attending school full time for his GED and attending a vocational school part time within the last 12 months, the client's participate in school will exceed 52 weeks and must be recorded.  If your client was not in school during the past 12 months, please indicate 52 weeks of not being in school of any kind.

 

For the list regarding Current Educational Status, check all that apply and write in average number of hours per week in each setting.

 

 

Does the client's performance meet developmental expectations?   Does the client have developmental delays?  Is your client's school performance comparable to his or her peers?

 

 

 

 

"Within the last four weeks" would include summer or other vacation time.

 

School Attendance

 

This is meant to measure truancy, not excused absences such as medical problems.

 

Most of the time: attends school four or more days per week

Sometimes: attends school between three and four days per week

Infrequent: attends school between one and three days per week

 

Grade Levels

 

Very Good: A- or better average

Good: B- to B+ average

Average: C- to C+ average

Below Average: D- to D+ average

Poor: F or Failing

 

Total number of suspension and expulsion DURING THE PAST 12 MONTHS

     -Indicate the number and not the days of suspension and/or expulsion.
